Eligibility Requirements
Before submitting a nomination, please ensure the nominee meets these requirements:
Service Requirements:
- Served honorably in the U.S. Armed Forces
- Demonstrated extraordinary valor or service
- Received honorable discharge or equivalent
Madison County Connection (one required):
- Resident for the past 3 years, OR
- Total of 5+ years Madison County residency, OR
- Born in Madison County or entered service as resident
Supporting Documents: Please attach DD-214, service records, awards, letters of recommendation, or other relevant documentation to strengthen the nomination.
